Custom Paint jobs?

I was looking at ChrisGuest picture of E39 http://myfdny.thebravest.com/photo/2967632:Photo:5798?context=user and was wondering who decides if a truck or engine is to a custom paint job? Also would anybody have a list of apparatus with custom paint jobs.

I'm sure the company decides as a whole to make modifications to their apparatus and I assume the captain has the final say (aside from a chief or other higher up saying "no").

As far as a list of unique paint schemes go, I know the following:

Engine 68/Ladder 49 (Yankees)

Engine 273/Ladder 129 (Mets)

Ladder 10

Squad 61

Not sure who's ex-engine that is in the picture (39 is using a spare) and if that company still has a custom paint job on their current engine.
